Science, technology, and innovation (STI)-oriented non-stock, non-profit organization (NSPO) including private and public foundations, and non-governmental organizations (NGOs) are certified for tax exemption purposes as mandated by the Republic Act (RA) No. 2067 (Science Act of 1958) and pursuant to RA No. 8424 or the National Internal Revenue Code and Tax Reform Act of 1997.

3. Certified true copies of the applicant’s most recent: a. General Information Sheet (GIS); b. Articles of Incorporation (AOI); c. By-Laws (BL); d. Certificate of registration of AOI and BL, and amendments, if there are any, issued by the Securities and Exchange Commission (SEC); e. Updated list of working staff;

4. Portfolio of S&T programs, projects, and activities (PPAs) with proposed budget for a period of three (3) years signed by the Executive Director or equivalent officer of the applicant foundation, accomplished using the DOST-approved and prescribed forms (SFU 001 and 002); https://sfu.dost.gov.ph/
5. Certification of funds availability to implement the submitted PPAs signed by the budget officer or any equivalent personnel, and the head of the organization; From applicant in any format/template
